Creating A New Structured Care Plan for a Resident

Care plans provide direction for individualized care of the resident. A care plan flows from each resident’s unique list of diagnoses and should be organized by the individual's specific needs. Continuity of care. The care plan is a means of communicating and organizing the actions of a constantly changing nursing staff. The new Care Plan module allows  to add structured Focus and a seamless link from the resident's Interventions to the resident's Nursing Instructions. 

 

  1. Resident Summary
  2. Open the Care Plans tab
  3. Add New
  4. To add a single Care Plan Focus from the library:
    1. Click Add From Library
    2. Search for the desired Care Plan by typing the name in the Focus field
    3. Click on the name of the desired Care Plan
    4. Add the Etiology, As Evidenced By and Additional Details
    5. Save
  5. After Care Plan Focuses are added, click on the name of the Care Plan Focus to open the care plan
  6. Add Goals for that Care Plan Focus
    1. Add New Goal - Add New Goal From Library
    2. Click Select next to each desired Goal(s)
    3. Add Selected
    4. Click on each added Goal (on the right) and edit the Goal details on the left
    5. Save
  7. Add Interventions for that Care Plan Focus
    1. Click on the intervention tab
    2. Click the Add New Intervention Button
    3. Select Add New Nursing Instruction Intervention
    4. Select the Nursing Instruction from the Question dropdown
    5. Enter/Update any necessary nursing instruction information
    6. Click Save
  8. Click "Activate" to activate the Care Plan Focus
